RESOLUTION AWARDING CONTRACT TO PROVIDER OF WOUND CARE SERVICES TO ID CARE, LLC, FOR RESIDENTS OF PREAKNESS HEALTHCARE CENTER, UNDER RFQ-24-078

WHEREAS, the County of Passaic (hereafter the “County”) is a government body incorporated pursuant to N.J.S.A. 40:18-1, et seq. and vested with all rights contained therein; and
WHEREAS, pursuant to N.J.S.A. 40:20-1 the Board of County Commissioners for the County of Passaic (hereafter the “Board”) are vested with decision-making powers on behalf of the County; and
WHEREAS, Preakness Healthcare Center (hereafter “Preakness”) is a New Jersey Department of Health licensed, 406-bed skilled nursing facility that is owned and operated by the Board, and is in need of Wound Care Services for residents of Preakness Healthcare Center; and
WHEREAS, pursuant to Resolution R-24-629, dated July 16, 2024, the Board of County Commissioners of the County of Passaic passed a Resolution qualifying a provider of Wound Care Services for residents of Preakness; and
WHEREAS, the Executive Director of Preakness Healthcare Center is requesting that the Board award a contract to ID Care, LLC, to provide Wound Care Services for residents of Preakness Healthcare Center; and
WHEREAS, no Certification is needed because the services are billed to a third party provider; and
WHEREAS, pursuant to N.J.S.A. 52:32-60-1, et seq., vendor executed the Non-Involvement in Prohibited Activities in Russia or Belarus Certification, attached hereto and made part of this Resolution, certifying the vendor is not involved in prohibited activities in Russia and Belarus, and the County confirmed vendor is not identified on the Specially Designated Nationals and Blocked Persons list created by the Office of Foreign Assets Control; and
WHEREAS, this matter was discussed at the September 3, 2024 meeting of the Health and Human Services Committee and is being recommended to the Board for approval; and
NOW, THEREFORE, LET IT BE RESOLVED, pursuant to N.J.S.A. 40A:11-5, et. seq., the Board of County Commissioners of the County of Passaic hereby authorizes an award of contract to ID Care, LLC, for the provision of Wound Care Services for residents of Preakness Healthcare Center during period July 1, 2024 to June 30, 2025, under RFQ-24-078; and
LET IT BE FURTHER RESOLVED, that the Director and Clerk are hereby authorized to execute any necessary agreements to effectuate the above.
